refactor(git): [CLAUDE] change commit message format

- Move [CLAUDE] tag from prefix to description
- Update format from '[CLAUDE] type(scope)' to 'type(scope): [CLAUDE]'
- Maintain Conventional Commit standard for tool compatibility
- Update all examples in CLAUDE.md, SKILL.md, and reference.md
- Update commit.sh to validate [CLAUDE] tag presence
- Add warning if [CLAUDE] tag is missing
